Excluded Providers

The Providers below may appear in the UHC Provider Search but are excluded from the AHS employee health plan as follows:

• Ascension St. John (OK) - all services excluded except true emergencies and mental health, alcohol or drug treatment – NEW 1/1/21


• Northwest Texas Healthcare System - all services are excluded except true emergencies and mental health, alcohol or drug treatment


• Preferred Imaging (TX) - all services are excluded – NEW 1/1/21


• Presbyterian Healthcare (NM) - all services excluded except true emergencies and mental health, alcohol or drug treatment


• St. Francis Health System (OK) - all services excluded except true emergencies, pediatrics, and mental health, alcohol or drug treatment – NEW 1/1/22


• CHRISTUS Trinity Mother Frances Health System is excluded from coverage except for emergency and NICU services for newborns under 34 weeks
  ***Including but not limited to Winnsboro, Good Shepherd/Longview, Tyler, Sulphur Springs, & Jacksonville locations


• Services at Texas Spine and Joint will be covered as out of network (based on plan elected), except for emergency services and Ear, Nose & Throat (ENT) procedures
