Refer to the upper-right corner on the front of your SutterSelect ID card to determine which plan option applies to you, and then click on the link corresponding to the network you wish to view:

SUTTERSELECT EPO PLUS
(Choose if Health Ventures, Inc., Bay Operating Unit, Valley Operating Unit or Sutter Health is in the upper-right corner on the front of the ID card.)

SUTTERSELECT PPO
(Choose if Health Ventures, Inc., Bay Operating Unit, Valley Operating Unit or Sutter Health is in the upper-right corner on the front of the ID card AND office visit copays mention Tier 1 and Tier 2.)

(Choose if SUTTER HEALTH NATIONAL is in the upper-right corner on the front of the ID card.)
