The Providers below may appear in the UHC Provider Search but are excluded from the AHS employee health plan as follows:
Ascension St. John (OK) - all services excluded except true emergencies and mental health, alcohol or drug treatment – NEW 1/1/21
Northwest Texas Healthcare System - all services are excluded except true emergencies and mental health, alcohol or drug treatment
Preferred Imaging (TX) - all services are excluded – NEW 1/1/21
Presbyterian Healthcare (NM) - all services excluded except true emergencies and mental health, alcohol or drug treatment
St. Francis Health System (OK) - all services excluded except true emergencies, pediatrics, and mental health, alcohol or drug treatment – NEW 1/1/22
CHRISTUS Trinity Mother Frances Health System is excluded from coverage except for emergency and NICU services for newborns under 34 weeks

Services at Texas Spine and Joint will be covered as out of network (based on plan elected), except for emergency services and Ear, Nose & Throat (ENT) procedures
